You have been sent this link to this survey because you have received support from South Lincolnshire CVS (SLCVS). Voluntary Action LeicesterShire (VAL) is providing an independent evaluation of the service and we want to find out your views.

We would like one survey completed per organisation or group. Information you provide will be analysed anonymously by VAL, and you will not be identified by name to SLCVS in any way.

The survey should take about 10 minutes to complete. Questions are multiple choice, with the opportunity to add comments if you wish and you can complete as much or as little as you like.

Please describe in your own words what changes there have been to your local community, service users or members as a result of the support (e.g. we have rented new premises and are now providing a new play group with 15 children attending a week / we have cleaned up part of the local park so that people are able to use it / we are providing new outreach debt advice sessions on a local estate used by around ten people a week).
